Pfizer COVID Clinic - Onondaga Nation Students 12+ Thu 5/27

The Onondaga Nation Health Clinic, in conjunction with Onondaga County, will be offering a Pfizer vaccine clinic for individuals ages 12 and up on Thursday May 27, 2021 from 12:00 PM - 2:30 PM at Tsha’ Thoñswatha - in the Community Room.

 

Vaccinations are open for Onondaga Nation residents ages 12 and up. 

 

1) Please call the Onondaga Nation Health Clinic at 315-469-6449 to make an appointment for the Pfizer Vaccine Clinic on Thursday May 27, 2021. Please call no later than Wednesday May 26th at 12:00 PM.

 

2) Each participant MUST bring a printed, completed consent form in order to be vaccinated. The consent form can be found at:

Printed copies will be made available through our next school material delivery to Onondaga Nation School students. If you need additional copies of the consent form, please contact the Onondaga Nation School main office at 315-469-6991.

 

3) An additional online form must also be filled out on behalf of the individual receiving the vaccine. Please use this link to access the form: http://forms.ny.gov/s3/vaccine.
